Payment<\/strong> for intake\/consultation is due prior to the appointment.<\/span>
Full payment<\/strong> for testing\/evaluation is due at the time of the first session.\u00a0<\/span><\/p>

Cash, checks,\u00a0and credit cards<\/strong>\u00a0accepted for intakes, consultations, and school observations.<\/span>

Insurance:<\/strong> <\/span><\/span><\/p>

I<\/strong> am an out-of-network provider<\/strong>. <\/span>I do not participate in any health insurance programs. However, after completion of the evaluation, I can provide you with a bill which supplies the procedure and diagnostic codes that can be submitted to your insurance company. There is no guarantee that your insurance company will reimburse you for these services. <\/span>Of note, insurance does not cover academic\/educational testing, school-based reports, or school observations, so this service would not be submitted toward your out-of-network benefits.\u00a0<\/span><\/p>

Patients\/parents should direct questions about out-of-network benefits, deductibles, and responsibility after deductible to their insurance company. A combination of the following CPT procedure codes are typically used for neuropsychological evaluations (96116, 96121, 96136, 96137, 96132, 96133)<\/em><\/span>
<\/em><\/strong>
<\/span>Independent Educational Evaluations<\/strong><\/span>:<\/strong><\/span> <\/span><\/p>

If an IEE is being conducted, then the school district must provide a NOREP and a commitment letter stating that they agree to pay Tarazi Neuropsychology Services the proposed amount upon completion of the evaluation.<\/p>

Legal fees and services:<\/strong><\/span>
Arrangement for payments for forensic\/legal services will be made directly with the attorney.<\/span><\/p>

Cancellations\/No-Shows:<\/strong> <\/span><\/p>

Although we recognize that circumstances change, we ask that you provide 48 hours\u2019 notice if you need to cancel\/reschedule an appointment for an evaluation\/testing. Same day no-shows for testing are charged a $300 fee.\u00a0<\/span><\/p>
