Payments/Insurance

Payment:

Payment for intake/consultation is due prior to the appointment.
Full payment for testing/evaluation is due at the time of the first session. 

Cash, checks, and credit cards accepted for intakes, consultations, and school observations.
Cash or check accepted for evaluations/testing. 

Insurance:

I am an out-of-network provider. I do not participate in any health insurance programs. However, after completion of the evaluation, I can provide you with a bill which supplies the procedure and diagnostic codes that can be submitted to your insurance company. There is no guarantee that your insurance company will reimburse you for these services. Of note, insurance does not cover academic/educational testing, school-based reports, or school observations, so this service would not be submitted toward your out-of-network benefits. 

Patients/parents should direct questions about out-of-network benefits, deductibles, and responsibility after deductible to their insurance company. A combination of the following CPT procedure codes are typically used for neuropsychological evaluations (96116, 96121, 96136, 96137, 96132, 96133)

Independent Educational Evaluations:

If an IEE is being conducted, then the school district must provide a NOREP and a commitment letter stating that they agree to pay Tarazi Neuropsychology Services the proposed amount upon completion of the evaluation.

Legal fees and services:
Arrangement for payments for forensic/legal services will be made directly with the attorney.

Cancellations/No-Shows:

Although we recognize that circumstances change, we ask that you provide 48 hours’ notice if you need to cancel/reschedule an appointment for an evaluation/testing. Same day no-shows for testing are charged a $300 fee.
